I am requesting a reasonable accommodation for an emotional support animal under the Fair Housing Act. I acknowledge that I am required to provide documentation from a licensed healthcare professional. I certify that the information I have provided is true and accurate.

I agree to provide consistent flea medications to prevent infestation. I agree to prevent my pet from roaming free on the grounds. Pet will be leashed or crated and in my presence at all times when leaving the apartment. Dog waste will be cleaned up immediately and disposed of in the dumpster. Cat litter or cage shavings will be sealed in a garbage bag for disposal. I also understand that the animal cannot pose a risk or cause disruption in any way to my roommates and neighbors. I will be held financially responsible for any damages to the apartment or premises by my ESA. Cameras are throughout the property and we will be monitored.
